Your child in year 8 is now due their HPV vaccinations in school as part of the National Childhood Immunisation Programme, please follow this link to complete an online consent for your child to receive these vaccines. If you would like to decline, please complete a no consent form on the attached link.

The vaccinations will take place at your child’s school on the 04/06/2026. The link will close on the 24/05/2026.

If your do not wish for your child to receive the vaccinations, please complete a ‘No’ to consent form.

Please be aware that if we do not receive the signed consent forms each young person will be assessed on an individual basis and given the opportunity to give their informed consent to receive the immunisations.
